Situation: 69 y/o male Michael Granger admitted from his cardiologist’s office for heart failure exacerbation.
Background: Diagnosed with stage ll right-sided heart failure last year and this is his first exacerbation. 10 lbs weight gain in three days, SOB, and worsening lower extremity edema.
Assessment: Patient has S3 heart sound, trace edema, and lungs are clear to auscultation.
