Think of a time in which you encountered a change in nursing practice, patient care delivery, or within a healthcare organization, and respond to the following:

What change did you encounter?

The change that I encountered, and I believe most of the nurses, have encountered is a change from paper charting to electronic charting or a change from one type of electronic medical record to another. We just went through a change that went live on May first with a hospice company that I work with as needed. That was from Axxess to what is called Homecare Homebase (HCHB).
